Documents required for electronic transfer of your policy benefits |

- Cancelled cheque of your bank account in which you wish to receive your policy payouts. Please ensure that the name of the account holder and the account number is clearly mention on the cheque.
- Signed copy of the Electronic payout form (link given below) with your bank account details
- Signed copy of your Photo Identity Proof, i.e PAN, Passport, Driving License

Email: Email the scanned copy to lifeline@iciciprulife.com from your registered email id. |

Once we receive your documents, your payout amount will automatically get transferred to your bank account within 15 days.
